The Office of the
Registrar maintains the academic records for current students and
alumni of Columbus State University.
How Long a Record is Retained . . .
Courses taken, grades assigned, and degrees earned at CSU become
part of the student's permanent academic record. Other academic
documentation and correspondence are retained by the Registrar for
seven years after the student's last term of enrollment at CSU.
How to Order an Academic Transcript,
Transient Permission Form, or Certificate of Enrollment . . .
Requests for academic transcripts
and transient letters are made to the Office of the Registrar and
require the student's signature. Students may make a request in writing by mail or fax using
one of the printable forms below. Transcripts may also be requested via ISIS by logging into
CougarNet at http://cougarnet.colstate.edu. See specific instructions for the
Transient Permission Form below.
Certificates of Enrollment
can be obtained directly from the National Student Clearinghouse
Self-Service Site via ISIS.

Academic Transcript

An academic transcript will not be issued if you have an outstanding obligation to CSU
(check ISIS for “Holds” on your record). There is no charge for transcripts. There is
a maximum of ten (10) copies issued per day. You are responsible for providing the recipient’s
name and address. Requests must include your signature, current address, social security number
and/or CSU student ID number and date of birth. For transcripts requested via ISIS, the
validation of your password is your signature.
Normal processing time after the receipt of a transcript request is two (2) business days.

Transient Permission Form

If you are a CSU student planning to enroll temporarily at
another institution and return to CSU, you must complete the
CSU Transient Permission Form. Print the form. Write clearly.
Complete all areas entitled To Be Completed By Student.
Deliver the form, along with a list of CSU courses that you
plan to take at the other institution, to the department chair
of your major. Ask your department chair to sign and deliver
the completed form to the Office of the Registrar, which will
verify the student information and mail the form to the
institution designated on the form and a copy to you.
There is no charge for processing transient letters. Normal
processing time after the receipt of a Transient Permission
Form in the Office of the Registrar is two business days.

Address/Data Change

Data changes (address, telephone number) for students enrolled
since Fall 1998 are made online via
ISIS.
Any student wishing to change his name and/or social security
number must submit a Name and Social Security Number Change
Form to the Office of the Registrar with the appropriate
documentation. |
