Telephone number--home, work or cell number

A check or money order made out to Columbus State University for the cost of the diploma.

 The cost to replace the diploma:

$25.00 Undergraduate

$25.00 Graduate degree (8 ½ x 11)

$40.00 Graduate degree (11 x 17) **

** This size diploma is not available to students who graduated from Columbus College (prior to Summer 1996)

NOTE:  The fee for the diploma must be submitted with your letter.  The diploma will be mailed within one to four weeks.
